Her Certificate of Baptism from December 5, 1903 does list her as "Blanche Clotilde Zajicek." She felt like she was just correcting the birth certificate. This tale was also related to Randy Blecha by Gertrude McDaniel Blecha. She lists herself as Catholic on the Vital and Personal & Statistics Form.
